Medically Unlikely Edits cap how many units of a code Medicare will pay on one line for one date of service. Know the MAI indicators and you will know which unit denials are appealable and which are final.
Four facts that decide whether your units get paid.
Each HCPCS or CPT code has a maximum unit value. Bill above it on a single line for a single date of service and the line is denied - no matter how real the services were.
Adjudication Indicator 1 means the edit applies per line. With documentation proving medical necessity for the extra units, you can appeal and win.
Indicator 2 is policy-based and absolute. It cannot be appealed with records - the limit is the limit, set by statute, regulation or CMS ruling.
Indicator 3 is based on clinical benchmarks rather than statute. Like MAI 1, it can be appealed with documentation showing the units were medically necessary.
The indicator tells you whether to fight the denial or fix the billing.
| MAI | Edit type | Appealable? | What to do |
|---|---|---|---|
| 1 | Line edit | Yes, with records | Appeal with documentation proving each unit was medically necessary and separately performed. |
| 2 | Date-of-service edit (policy) | No | Do not waste appeal dollars. Fix the unit count or split services across dates when clinically appropriate. |
| 3 | Date-of-service edit (clinical) | Yes, with records | Appeal with clinical notes showing why the case exceeded the benchmark - severity, complexity, complications. |
