Beyond the Code

Knowing the Code Is Step One

The right code on the claim is only half the battle.

Specificity Wins

Unspecified codes pay less attention and more denials. Code to the highest specificity the documentation supports.

Sequence Matters

First-listed diagnosis drives medical necessity. Put the reason for the visit first, not the most expensive condition.

Annual Updates

ICD-10 updates every October 1. Coding from last year's book is a fast path to invalid-code rejections.

Zaxis Coding Review

Our coders validate diagnosis coding on every audit - specificity, sequencing and LCD coverage before the claim goes out.

FAQ

Questions, Answered

Straight answers, no jargon.

How many ICD-10 codes are there?
Over 70,000. This lookup covers the most commonly used codes across primary care and major specialties - the ones that drive most claim volume.
What is the difference between ICD-10-CM and ICD-10-PCS?
ICD-10-CM is the diagnosis code set used on all claims. ICD-10-PCS is the procedure code set used only for hospital inpatient procedures. This tool covers ICD-10-CM diagnosis codes.
When should I use a Z code?
Z codes describe encounters and health status, not diseases - screenings (Z12.11), immunizations (Z23), long-term medication use (Z79.x). Sequence them correctly: screening Z codes go first for screening encounters.
What does seventh character mean in ICD-10?
Injury codes (S and T chapters) need a 7th character for encounter type: A initial, D subsequent, S sequela. Missing 7th characters are a top rejection cause in orthopedics and urgent care.
Why was my claim denied for diagnosis?
Usually one of three reasons: the code was not specific enough, the diagnosis does not support the procedure under LCD rules, or the code was invalid for the date of service.
